From time to time, SCCC may use photo / video recording in our services. Your entry into our facilities signifies consent to the possibility that you / your child's image may be used in print or electronic promotion. Please check yes or no if you do or do not want your child to be photographed or video recorded by SCCC.

We use flashing lights, low lights, and loud music during our worship and play times to enhance the experience your child will have in Kids Dive. Please check yes if your child is sensitive to any of these types of activities or experiences, or no if your child is not sensitive.
